DSWD MEDICAL CASH AID REQUIREMENTS – Here are the documents required and the process on how to apply for the aid.
In applying for the DSWD Medical Cash Aid, there are requirements that you need to prepare as well as a process that must be observed. Meanwhile, the application process involves only three (3) steps.
Step 1 — Screening
The first step that you need to take in applying for the Medical Assistance offer of the social welfare department is to file the required documents. Here are the documents that the DSWD requires in applying for the aid depending on which suits your case:
- Medical Certificate form a Licensed Physician
- a Discharge Summary Form the Hospital
- Death Summary if the patient passed away
- Final Hospital Bill
- Statement of Account from the hospital
- Certificate of Balance from the hospital
- Doctor’s Prescription

Furthermore, there are also additional requirements in the case of therapy and other treatments. You need to submit the following to the DSWD:
- Treatment Protocol
- PhilHealth Certificate
- Quotation including the full amount to be paid, must bear signature of the issuing officer
- Psychiatrist Certification indicating the doctor’s signature and the license number
Step 2 — Assessment
After submitting the requirements to the Department of Social Welfare and Development, the government agency will assess the application to determine the case’s qualification for the aid. The social welfare staff will assess the financial status and the health condition of the patient.
Step 3 — Payout
For cases eligible under the DSWD Medical Cash Aid grant, the next step will be the payout. The social welfare institution will release the money once you have complied with the requirements for the releasing.
