- Department of Trade and Industry (DTI) Registration (if Sole Proprietorship)
- Articles of Incorporation with By Laws and General Information Sheet (GIS) (if (Corporation)
- Certificate of Incorporation (if Corporation)
- Latest BIR stamped Income Tax Return (ITR) or Form 1701 with Audited Financial Statement
- Trade Reference or List of Customers and Suppliers with Name, Office Number and Office Address
For Overseas Filipino Workers (OFWs):
