Overseas Filipino Worker (OFW)
- two (2) valid government issued ID’s with photo and signature
- Latest Proof of Billing under your name
- Proof of Remittance(allotment slips) for the last 3 months
- Latest Crew Contract duly authenticated by Phil. Consulate (for OFWs)
- Consularized Special Power of Attorney
