- SSS ID
- GSIS ID
- PRC ID
- Driver’s License
- Passport
- TIN (plastic card type)
- NBI Clearance
- Integrated Bar of the Philippines ID
- For employed applicants:
- photocopy of BIR form 2316
- For self-employed applicants:
- photocopy of BIR form 1701
- audited Financial Statements with BIR stamp for the past 2 years
- For credit-card holders:
- original copy of the latest credit card billing statement (with the client as the PRINCIPAL cardholder)
