- Mortgage Redemption Insurance (MRI) Application Form
- Proof of Billing/IDs with address
- PSA copy of CENOMAR if single; PSA copy of Marriage Certificate if married; PSA Copy of MC with nullity of Marriage and PSA CENOMAR if annulled/divorced; Marriage Certificate with annotation of finality of legal separation if separated with spouse; copy of Death Certificate of spouse and PSA CENOMAR if widowed
- Special Power of Attorney (SPA)
- Authorization Letter Template for Property Collateral Verification and Tax Mapping
- proof of income (any 2 of the following):
- Payslip [latest three (3) months]; or Bank Statement [latest three (3) months]
- Certificate of Employment with compensation [issued within the last six (6) months]
- Latest crew contract (for seafarer/ seaman)
- Latest employment contract (for land-based OFWs)
- Allotment slip
- Sea Service Certificate (for seafarer)
You also need to include the collateral requirements or the documents pertaining to the property. Prepare the following:
General Requirements:
- copy of collateral title (TCT/CTC)
- copy of tax declaration on land and/or improvement
- appraisal fee paid to SBC: OR Number
- tax map verification with one valid ID of the registered owner
House Construction or Renovation
- Bill of materials and estimates of construction and labor cost
- Floor and building plan and building specification duly signed by the civil engineer/architect
Acquisition from Accredited
- Contract to sell or reservation agreement
Loan Take-Out / Home Refinancing
- proof of amortization for the last three (3) months
- Statement of account from the
- mortgagee bank
