For Self-Employed Applicants:

 - Valid Government Issued Identification Card for nominated Attorney in Fact (AIF) (if applicable)
 - Valid Alien Certificate of Registration (ACR) with permanent residency status and official receipt (if applicable)

- Business Permit
 - Securities and Exchange Commission (SEC) Registration
 - Department of Trade and Industry (DTI) Registration (if Sole Proprietorship)
 - Articles of Incorporation with By Laws and General Information Sheet (GIS) (if (Corporation)
 - Certificate of Incorporation (if Corporation)
 - Latest BIR stamped Income Tax Return (ITR) or Form 1701 with Audited Financial Statement
 - Trade Reference or List of Customers and Suppliers with Name, Office Number and Office Address
