List of PWD ID Requirements for Application Process
PWD ID REQUIREMENTS – Here are the documents needed in applying for the Persons with Disability (PWD) ID card.
In the Philippines, there are special privileges for individuals who have disabilities provided that the person has a PWD ID. It is issued by the Persons with Disability Affairs Office or the city/municipal Social Welfare and Development Office where the person resides.
The PWD ID can be used in availing discounts in purchases and services accessed. Most establishments require the presentation of the ID prior to granting discounts or another privilege set for the cardholders.
In applying for the said ID, there are PWD ID requirements for application. Here are the documents that you need to prepare and submit in applying for the PWD ID:
- duly-accomplished PWD ID Application Form which is available from any of the following:
- Department of Social Welfare and Development (DSWD) offices
- Office of the Mayor
- National Council on Disability Affairs (NCDA) or its regional counterpart
- DOH Online Registration System
- Participating organizations with memorandums of agreement with the DOH
- PWD ID application form from the DOH website
- Office of the Barangay Captain
- Two (2) recent 1×1 ID photos with name and signature or thumb marks at the back
- any valid government-issued ID
- latest school assessment/medical certificate/or disability certificate confirming disability condition from government facility or licensed private clinic or medical professional
Getting the PWD ID if free of charge on the person’s first time to apply for it. The PWD ID is vaid for five (5) years.
Meanwhile, the cardholder has an option to renew the card after five (5) years to maintain the access to the benefits, discounts, and other privileges set for the cardholders. Following the first expiration, there will be minimal charges for renewal.
