REQUIREMENTS FOR DSWD SOLO PARENT ID – Here are the documents needed in applying for the DSWD Solo Parent ID application.
The Department of Social Welfare and Development, one of the government agencies that has assistance offers for solo parents, offers a Solo Parent ID that can qualify its cardholders in availing several assistance offers and benefits crafted by the government entity.
In availing the DSWD Solo Parent ID, there are requirements that you need to comply. These requirements include both eligibility criteria and documentary requirements. To qualify for the ID, the applicant must meet the following:
Eligibility Requirements:
- solo parents who were left alone to do the parenting job due to:
- death of spouse
- spouse detention or serving a sentence for a criminal conviction for at least one (1) year
- mental incapacity of the spouse
- physical incapacity of the spouse
- legal separation
- declaration of nullity of marriage
- woman whose pregnancy is not intended
- unmarried father or mother who chose to raise their child independently without entering marriage
- any member of the family who assumed the role as the head of the family after the parents or single parents vanished
- any person who provides parental care to a child/children
As mentioned earlier, there are also documents that must be complied by the applicant for the DSWD Solo Parent ID in applying for the identification card. Here are the documentary requirements for the DSWD Solo Parent ID application:
Documentary Requirements:
- Barangay Certificate which can be obtained from the Barangay Hall
- proves your residency to your barangay for at least six (6) months already
- confirms the single status
- PSA Birth Certificate of Child/Children obtained from the Philippine Statistics Authority office
- Proof of Financial Status
- Income Tax Return (ITR)
- Certification from employer stating income
- Certification from Barangay Captain
- must indicate status as solo parent
- duly-accomplished DSWD Solo Parent ID Application Form
- supporting documents / certificates
- Declaration of Nullity of Marriage
- Certificate of No Marriage (CENOMAR)
- spouse’s Death Certificate
- Medical Certificate proving mental incapacity of spouse
- jail records / Certificate of Detention
- VAWC Report in case of abandonment
