1. Referral Slip or Certificate

4. Court Order or Subpoena

5. Death Certificate
You may also apply for the Transportation Cash Aid to attend the burial of a family member most especially in the case of urgency to get to the place. A Death Certificate may be presented to the social welfare department to avail the aid.
