PAG-IBIG HELPs REQUIREMENTS – For self-employed members of the Fund who want to apply for the Pag-IBIG Fund Health and Education Loan programs, here are the documents required for submission.
Do you have pending goals that may be addressed by health and education programs that can provide you some financial solutions? If you are a member of the Pag-IBIG Fund, you have something to turn to.
While the main Pag-IBIG loans already do a lot for the members of the Fund, the state entity did not stop from growing its offers. Currently, its loan offers already include the Pag-IBIG Fund Health and Education Loan programs or also called the Pag-IBIG HELPs.
Through the Pag-IBIG HELPs, you can push through with your health and education goals and earn discounts in the partner schools and hospitals of the Fund. Part of the Pag-IBIG HELPs requirements are the qualifications in applying for loan programs which are the following:
- an active Pag-IBIG member with at least one (1) monthly contribution posted within the last six (6) months
- with at least 24 monthly membership savings under the Pag-IBIG Regular Savings
- avail the loan from Pag-IBIG HELPs partner schools and/or hospitals
If the member-borrower has an existing Pag-IBIG Multi-Purpose Loan, House Loan, and/or Calamity Loan, the acount must not be in default. The loan programs are not only open for locally-employed but as well as self-employed individuals.
Here are the documentary requirements in applying for Pag-IBIG HELPs if you are a self-employed individual:
- duly-accomplished Pag-IBIG HELPs Application Form
- billing assessment from the HELPs partner school or hospital
- one (1) valid ID
- Philippine Identification (PhilID) Card
- Passportincluding those issued by foreign governments
- Driver’s License
- Professional Regulation Commission (PRC) ID
- National Bureau of Investigation (NBI) Clearance
- Police Clearance
- Postal ID
- Voter’s ID
- Tax Identification Number (TIN) ID
- Barangay Certificationor Barangay IDs or similar documents bearing picture of the Member
- Government Service Insurance System (GSIS) e-Card
- Social Security System (SSS) Card
- Senior Citizen Card
- Overseas Workers Welfare Administration(OWWA) ID
- Overseas Filipino WorkerID
- Seaman’s Book or Seafarer’s Identification and Record Book (SIRB)
- Alien Certification of Registration/Immigrant Certificate of Registration
- Government Office and GOCC ID, e.g. AFP ID, Pag-IBIG Fund Loyalty Card
- ID issued by the National Council on Disability Affairs (NCDA)
- Department of Social Welfare and Development (DSWD) Certification
- Integrated Bar of the Philippines ID
- Company ID issued by Private Entities or Institutions Registered with or supervised or regulated either by the BSP, SEC or IC
- PhilHealth Health Insurance Card ng Bayan
The Fund requires a proof of income. In the case of self-employed member-borrowers, you may submit any of the following:
- ITR, Audited Financial Statements, and Official Receipt of tax payment from bank supported with DTI Registration and Mayor’s Permit/Business Permit
- Commission Vouchers reflecting the issuer’s name and contact details (for the last 12 months)
- Bank Statements or passbook for the last 12 months (in case income is sourced from foreign remittances, pensions, etc.)
- Copy of Lease Contract and Tax Declaration (if income is derived from rental payments)
- Certified True Copy of Transport Franchise issued by appropriate government agency (LGU for tricycles, LTFRB for other PUVs)
- Certificate of Engagement issued by owner of business
- Other document that would validate source of income
