- Original or Certified True Copy of the updated Clinical Abstract or Medical Abstract signed by the attending physician with license number.
- Any valid ID (patient and representative)
In case there is no immediate relative of the patient who is available for interview with the PCSO, an authorization letter must be submitted in applying for medical assistance.
