Guide on the Requirements for CHED Medical Scholarship Offer Application
REQUIREMENTS FOR CHED MEDICAL SCHOLARSHIP – Here are the eligibility and documentary requirements in applying to be a scholar.
Truth be told that medical courses are among the most expensive courses to take in the Philippines. That’s why even though a lot of people aspire to become nurses and doctors, these courses seem really not for everyone who want it.

There are both eligibility and documentary requirements for CHED Medical Scholarship application. For the eligibility criteria, to qualify to apply as a scholar, the student applicant must:
- be an incoming freshmen college student
- be a Filipino citizen
- have a GWA of at least 90% in high school or the equivalent for freshmen applicants
- not have a failing grade in any subject
- not be a recipient of any other scholarship or financial assistance program
- not have been enrolled in any undergraduate or graduate program before
- be of good moral character
- be in good health
- not have any pending disciplinary action
With regards to the documentary requirements for CHED Medical Scholarship application, here are the documents that you must prepare:
- Certified true copy of high school diploma or equivalent
- Certified true copy of Form 138 or report card
- Certificate of Good Moral Character from the high school principal or guidance counselor
- Birth certificate
- Medical certificate from a licensed physician
- Certificate of Indigency (if applicable)
- Certificate of Non-Availability of Parents’ Income Tax Return (if applicable)
- Two recent 2×2 ID pictures
