  • School ID / Student ID — The social welfare department said that the school ID must be presented for the purpose of verification of the enrollment of the student applicant in a school or educational facility. The school ID presented must be valid and must have been issued by the educational facility or school where the student is currently enrolled.
  • Student Fee Slips — This document is used in the assessment of how much financial assistance does a student need from the government office.
  • Enrollment Form or Registration Document — Also called the E.F., it is another proof of the enrollment of the student in the school or educational institution.
  • Name of School — The student must indicate the full name of the school or educational institution for the purpose of verification.
  • Parents / Guardians ID — The ID of the parents or guardians is needed as proof that the parent or guardian consent the application of the child to the Educational
