Guide on UMID Application & the Documents You Must Prepare

One of the common requirements in applying for a loan, claiming a benefit offer of a government agency, booking a ticket, applying for a passport, etc. is a valid ID. The Unified Multi-Purpose Identification or UMID ID is the single identification card for the Social Security System (SSS), Government Service Insurance System (GSIS), Home Development Mutual Fund (HDMF or Pag-IBIG Fund), and Philippine Health Insurance Corporation (PhilHealth) members.

Step 1 — Check if you are qualified to apply for the UMID ID.
The Social Security System (SSS) has set eligibility requirements in applying for the unified ID. To qualify for it, you must meet the following eligibility criteria:
- active SSS account
- has posted at least one (1) monthly contribution to SSS
Step 2 – Prepare the requirements.

Step 3 – Apply for the UMID ID.
The UMID ID application can be done in two (2) ways — directly going for an over-the-counter application at a branch of the Social Security System (SSS) nearest to you or booking an online appointment first through the My.SSS portal. However, it is important to know that some SSS branches really require an online appointment.
If you want to make a personal application at an SSS branch, here are the steps:
- Prepare the documentary requirements and go to the SSS branch nearest to you.
- Ask for a copy of the UMID Application Form and provide the details requested on the form.
- Submit the duly-accomplished form and the other requirements to the SSS staff.
- Wait for your application to be verified.
- Proceed for the taking of your photo, electronic signature, and fingerprint scan.
- Wait for the release of the small slip which you will use in claiming the ID.
With regards to booking an appointment first online before going at an SSS online portal, here are the steps that you may follow:
- Prepare the documentary requirements.
- Secure an online copy of the UMID ID application form. Write in capital letters and state N/A if the requested information in the form is not applicable to you.
- Go to My.SSS online and log in to your account.
- Provide the required information and pick the time for your appointment.
- Go to the SSS branch where you made an appointment.
- Ask for a copy of the UMID Application Form and provide the details requested on the form.
- Submit the duly-accomplished form and the other requirements to the SSS staff.
- Wait for your application to be verified.
- Proceed for the taking of your photo, electronic signature, and fingerprint scan.
- Wait for the release of the small slip which you will use in claiming the ID.
