UMID ID REQUIREMENTS – Here are the eligibility criteria and documents needed in getting a Unified Multi-Purpose Identification card.

One of the most popular and flexible IDs in the Philippines is the Unified Multi-Purpose Identification card or better called as the UMID ID.

The UMID ID is the single identification card for the members of the Government Service Insurance System (GSIS), Social Security System (SSS), the Pag-IBIG Fund, and the Philippine Health Insurance Corporation (PhilHealth).

SSS

The UMID ID is requested from the Social Security System or SSS, one of the biggest state-run social insurance institutions in the country. It replaced the SSS ID and is usually released within 30 days after the application was filed at the state-run social insurance agency.

Eligibility Criteria. The Social Security System (SSS) has set qualifications in applying for the Unified Multi-Purpose Identification (UMID) ID. Here are the qualifications:

  • active SSS account
  • has posted at least one (1) monthly contribution to SSS

You can apply for a new UMID ID if you have yet to get yours or a replacement in case your ID is old or was lost, stolen, or damaged. You may also request for a new ID in case you want to update your details like the change of name or making corrections in your personal data.

UMID ID Requirements. With regards to the documents that you need to present in getting a Unified Multi-Purpose Identification (UMID) ID, you need to submit at least one (1) valid ID or two (2) secondary IDs in original copies. You can choose from the following:

Primary IDs:

  • Unified Multi-Purpose ID (UMID) Card (for replacement or upgrading)
  • Postal Identity Card
  • Social Security (SS) Card
  • Passport
  • Driver’s License
  • National Bureau of Investigation (NBI) Clearance
  • Voter’s ID Card
  • Seafarer’s Identification and Record Book (Seaman’s Book)
  • Alien Certificate of Registration
  • Firearm Registration
  • License to Own and Possess Firearms
  • Permit to Carry Firearms Outside of Residence

Secondary IDs:

  • School or Company ID
  • Senior Citizen Card
  • Taxpayer’s Identification Number (TIN) ID
  • Pag-IBIG Member’s Data Form (MDF)
  • PhilHealth Member’s Data Record (MDR)
  • Birth Certificate
  • Marriage Contract
  • Police Clearance
  • Overseas Workers Welfare Administration (OWWA) Card
  • Baptismal Certificate
  • Transcript of School Records
  • Permanent Residency ID
  • Health or Medical Card
  • Bank Account Passbook
  • Credit Card
  • ATM Card with Cardholder’s Name
  • Life Insurance Policy

  • ID Card issued by the LGU (Barangay/Municipal/City)
  • ID Card issued by a professional association recognized by the Professional Regulation Commission (PRC)
  • Fisherman’s Card issued by the Bureau of Fisheries and Aquatic Resources (BFAR)
  • Temporary License issued by the Land Transportation Office (LTO)
  • Seafarer’s Registration Certificate issued by the Philippine Overseas Employment Administration (POEA)
  • Certificate of Naturalization from the Bureau of Immigration
  • Certificate from the Office of Muslim Affairs
  • Certificate from the Office of Northern/Southern Cultural Communities

You may apply for your UMID ID at the nearest branch of the Social Security System (SSS) or online through the My.SSS platform.
