REQUIREMENTS FOR DSWD 4PS APPLICATION – Here are the documents needed in applying to be a beneficiary of the DSWD 4Ps.
One of the biggest anti-poverty programs in the Philippines is the Pantawid Pamilyang Pilipino Program or more commonly known as 4Ps. It is headed by the Department of Social Welfare and Development (DSWD) and was crafted under a law.
With 4Ps, the national government wants to be able to provide assistance to the poorest of the poor Filipinos. There are eligibility requirements as well as documentary requirements for DSWD 4Ps application and you can check on them below if you are planning to apply to be a beneficiary.
Factors Considered by DSWD in Choosing the Recipients / Beneficiaries:
- a resident of one of the poorest communities in the Philippines
- having children between 0 and 18 years of age or a pregnant member of the family when the assessment was made
- a part of the household whose economic status is below the provincial poverty level
List of Requirements for DSWD 4Ps Program Application:
- copies of birth certificates of all family members
- copy of health records of 0-5 years old member of the family from the health center where they have their check-ups
- barangay certificate
- 2 1×1 picture with white background
- copies of school IDs or first grading period school report cards of all children in the family who attended school
- valid IDs of the family member attending the community assembly
