The assistance of the DSWD for persons with disabilities or PWDs can be availed when purchasing items from various establishments or buying certain products.
Who are qualified to apply for assistance to DSWD?
The DSWD cited that PWDs are those people who are suffering from restrictions due to a mental or sensory impairment, male or female, and aged 0 to 59. The government has cited those who are physically disabled and those who are mentally disabled.
Physically Disabled:
- hearing impaired
- visually impaired
- orthopedically disabled
- other physical disabilities (cleft palate, hareclip which affects their speech)
Mentally Disabled:
- mentally retarded
- improved mental patients
- attention deficit disorder
- autistic
- attention deficit hyperactive disorder
How to Apply for DSWD PWD Card?
- Step 1 – Go to the main office or branch of DSWD nearest to you and ask for a copy of the PWD ID application form. Fill in the form with the needed information.
- Step 2 – Prepare your two (2) recent 1×1 ID photos with name and signature or thumb marks at the back.
- Step 3 – Secure the latest school assessment/medical certificate/or disability certificate that confirms disability condition from licensed private or government clinic, or hospital-based medical professional.
- Step 4 – File your application to the DSWD office.
