- Duly-accomplished Application for Provident Benefits (APB) Claim Form
- Pag-IBIG Loyalty Card
- One (1) Valid Identification Card (ID)
According to Pag-IBIG Fund, the ID must be presented in original and in photocopy. In the absence of a loyalty card, the member must present two (2) valid IDs instead of one (1).
In the case of retirement purposes, the IDs that must be presented must indicate the member’s date of birth. If there is no available ID with date of birth, any of the following may be presented:
- PSA-Issued Birth Certificate; or
- Notarized Non-availability of Birth Record issued by PSA and Joint Affidavit of Two Disinterested Persons
