SSS MATERNITY CLAIM REQUIREMENTS 2024 – Here’s a guide on the documents that qualified members of the SSS must prepare and submit.

Are you one of the millions of members of the Social Security System? In the Philippines, it is one of the biggest state entities with several members coming from different categories — individuals employed in the private sector, overseas Filipino workers (OFWs), self-employed individuals, and a lot more.

To maintain an updated account, an SSS member must regularly post a contribution. On the other side of an updated account is the possibility of qualifying for the SSS loans and the benefit offers. One of the benefit offers is the Maternity Benefit.

The SSS Maternity Benefit or also called the SSS Maternity Claim was crafted to assist the female members of the social insurance institution in times of pregnancy. To qualify for the benefit offer, the member must inform her employer (if locally-employed) or the SSS directly about her pregnancy within 60 days since the concepction.

In availing the benefit offer, there are SSS Maternity Claim requirements that a member must prepare and submit to the social insurance entity. Here are the documents required by the state institution:

If employed:

  • Maternity Notification Form duly stamped and received by SSS
  • Maternity Reimbursement Form
  • UMID or SSS biometrics ID card or two (2) other valid IDs, both with signature and at least one (1) with photo and date of birth. 

If self-employed or voluntary member:

  • Maternity Notification Form duly stamped and received by SSS
  • Maternity Reimbursement Form
  • UMID or SSS biometrics ID card or two (2) other valid IDs, both with signature and at least one (1) with photo and date of birth

If separated from employment:

  • Maternity Reimbursement Form
  • Certification from the last employer showing the effective date of separation from employment or notice of company’s closure/strike or certification from the Department of Labor and Employment that the employee or employer has a pending labor case.
  • Certification that no advance payment was granted (if confinement days applied for are within or prior to separation).
  • UMID or SSS biometrics ID card or two (2) other valid IDs, both with signature and at least one (1) with photo and date of birth

Aside from the aforementioned documents, there are documents required by the SSS for submission based on the type of delivery:

  • For normal delivery — Certified true or authenticated copy of the duly registered birth certificate
  • For caesarean delivery — Certified or authenticated copy of the duly registered birth certificate and certified true copy of operating room record/surgical memorandum
  • For miscarriage:
    • obstetrical history stating the number of pregnancies certified by the attending physician and dilatation and curettage (D&C) report
    • pregnancy test before and after abortion with the age of gestation
