The SSS Maternity Benefit is just one of the many SSS benefits and loan offers of the social insurance giant Social Security System. The benefit offer is also called the SSS Maternity Claim. Under it, a member may avail an amount that can aid her and her partner in the pregnancy or childbirth expenses.

SSS Maternity Claim Requirements

However, it is important to note that there are SSS Maternity Claim requirements that you must prepare and submit to the Social Security System in applying for the benefit offer. The documentary requirements may vary on a case-to-case basis.

Here is a guide on the SSS Maternity Claim requirements for application:

In cases of live childbirtth, to apply for an SSS Maternity Claim to the Social Security System system, submit any one (1) of the following documents:

  • Child’s Certificate of Live Birth duly registered with the Local Civil Registrar (LCR) or issued by the Philippine Statistics Authority (PSA); or
  • Report of Child’s Birth issued abroad; or
  • Child’s Certificate of Death duly registered with the LCR or issued by the PSA or issued abroad.

In case the document is issued abroad, it must be submitted with an English translation if applicable.

If Miscarriage, submit any one (1) of the following documents that prove the pregnancy:

  • Result of pregnancy test duly signed by the physician/municipal health officer; or
  • Result of other diagnostic test which may include ANY of the following:
    • Ultrasound; or
    • Blood Pregnancy test (Beta HCG); or
    • Early Pregnancy factor

If H-Mole/Ectopic Pregnancy, submit a Certified true copy of hospital/medical records stating how the condition was managed.

If Still Birth/Fetal Death, submit any of the following, whichever is applicable:

  • Fetal Certificate of Death duly registered with the LCR or issued by the PSA or issued abroad; or
  • Certified true copy of hospital/medical records stating the outcome of delivery.

There are additional documents you may submit in certain cases like in the case of solo parents. Here are these special cases and the supporting SSS Maternity Claim requirements you may submit:

If Solo Parent, submit any of the following documents, whichever is applicable:

  • Solo Parent ID issued within two (2) years from date of delivery by the Local Government Unit (LGU) and signed by the Social Worker and the City/Municipal Mayor; or
  • Certification of eligibility of the Solo Parent issued by the LGU and signed by the Social Worker/City/ Municipal Mayor, if the ID is not yet available.

If Caesarian Section Delivery, submit any of the following documents on top of your SSS Maternity Claim requirements:

  • Operating Room Record (ORR); or
  • Discharge Summary Report; or
  • Surgical Memorandum; or
  • Detailed invoice showing caesarian delivery charges, for deliveries abroad only; or
  • Medical/Clinical Abstract; or Delivery Report; or
  • Similar medical documents submitted which indicate the type of contingency.

For SE/VM who was previously employed or a member separated from employment, in case the delivery, miscarriage, or the ETP happened within the employment period or within six (6) months from her date of separation from employment, the SSS member must submit any of the following, whichever is applicable:

  • Certificate of Separation from Employment indicating the effective date of separation and that no advance payment was granted by the Employer; or
  • Affidavit of undertaking indicating effective date of separation from employment and that no advance payment was granted by the Employer.
