• For Self-Employed Applicants:
  • Information on other credit card account/s with at least one (1) year principal card membership (MANDATORY);
  • Latest BIR Form 1701
  • Audited Financial Statement Received by Bureau of Internal Revenue (BIR)
• For Non-Immigrants and Embassy Employees
  • Photocopy of unexpired passport or visa
