- Customer Information Sheet (CIS) for Individuals/Sole Proprietor
- Signature Card
- Terms and Conditions and Annex to Terms and Conditions for Deposit Products and Services
- Customer Privacy and Data Protection Notice
- Account Information Card
- For ATM-based accounts: ATM Card Application and Maintenance Form
For Checking accounts: Checkbook Control Card
Identification Documents:
- Latest 2×2 or 1×1 colored ID picture
- Any Government-issued IDs
- Proof of Address
- Latest Utility Bill
- Credit Card Bill
Aside from the aforementioned requirements, the applicant must also prepare an amount for the UCPB initial deposit based on the account he or she wants to open.
