Step 1 — Log in to www.register.philsys.gov.ph and provide your demographic information to set an appointment. Fill out all the required fields and submit your application form. Save a copy of the Application Reference Number (ARN) and/or your QR code.
Step 2 — Go to your designated registration center on your appointment schedule. Bring your PhilSys ID requirements for application, your ARN, and/or your QR code. You may also bring any of the following supporting documents:

- Pantawid Pamilyang Pilipino Program (4Ps) ID
