DSWD CASH ASSISTANCE REQUIREMENTS – Here are the documents needed in applying for cash assistance to DSWD in times of crisis.
In times of crisis, the poorest of the poor in the Philippines may turn to the Department of Social Welfare and Development (DSWD). It is one of the biggest government agencies in the country that runs several programs that were crafted to help the poor Filipinos.
One of the programs under the DSWD is the Assistance to Individuals in Crisis Situations or more commonly known as AICS. Under it, it has a cash loan offer for poor Filipinos in times of crisis.
The cash assistance offer for DSWD services four (4) main purposes – medical assistance, transportation assistance, educational assistance, and burial assistance. There are different documents that you must prepare and submit to apply for the cash aid.
For medical assistance or hospitalization purposes:
- Clinical Abstract and/or
- Barangay Certification/Indigency/ any valid ID of the patient or his/her representative
- Referral/Endorsement letter from legislator, if applicable
- Social Case Study Report from LGU
- Medical Certificate with license/PTR No. of the attending physician
- Laboratory Request/Medical Procedures (recommended by the attending Physician)
- Billing Statement (for hospital bill)
- Prescription (for medicine)
For burial assistance:
- Registered Death Certificate
- Funeral Contract
- Social Case Study report from LGU (optional)
- Barangay Certificate/any valid ID
- Referral/Endorsement Letter from legislator
- Permit to Transfer
For transportation assistance:
- Barangay Certification/Indigency/any valid ID
- Social Case Study Report from LGU (optional)
- Referral/Endorsement letter from legislator, if applicable
For educational assistance:
- Barangay Certification/Indigency/any valid ID
- Social Case Study Report (optional)
- Certificate of Enrollment
- Referral/Endorsement letter from legislator, if applicable
