  • 2 valid Ids

  • Police Clearance
  • NBI Clearance
  • Postal ID

  • Driver’s License

Acceptable Proof of Income:

  • For Employed – Certificate of Employment (COE) and Payslip indicated your salary
  • For Self-Employed – Mayors Business Permit or Brgy Business Permit or Picture of Business with Declaration of Income
  • For Recipient of Remittance – 3 months latest remittance slip or soa/bank statement
  • Pensioner -Statement of Account/Bank statement
  • For OFW/Seaman – Return Plane Ticket or Latest Contract
