- Option 4: 6 months old Certificate of Employment w/ Compensation and Benefits signed by Authorized Signatory
- For self-employed applicants, proof of business such as any of the following:
- Option 1: LATEST 3 Years of ITR and LATEST DTI/SEC Registration
- Option 2: LATEST 3 Years of Audited Financial Statements
