A member of health insurance agency must submit a premium to be eligible to avail the PhilHealth benefits when the medical emergencies, even non-emergency ones, take place. You may pay for the contribution monthly or quarterly depending on your choice.

With regards to applying for a PhilHealth membership, there are two (2) main ways — personally at a branch office of PhilHealth or online through email. Before discussing the registration process, make sure you have the following PhilHealth requirements:

  • duly accomplished Philhealth Member Registration Form (PMRF)
  • Two copies of latest 1×1 ID pictures
  • at least two valid IDs
  • PSA-issued Birth Certificate (photocopy)
  • Marriage Certificate (if applicable)
  • PSA-issued Birth Certificate of qualified dependents

With regards to the process in applying for a PhilHealth membership, the PhilHealth requirements are the same but the process is different. It depends whether you are employed, self-employed, a senior citizen, or unemployed or applying for a voluntary membership.

For Employed Applicants

  • Step 1 – Prepare the documents needed in getting a PhilHealth ID.
  • Step 2 – Fill out the two (2) copies of the PhilHealth Member Registration Form (PMRF)
  • Step 3 – Submit the PMRF to your HR Department. The employer or company will process the application for PhilHealth ID.
  • Step 4 – Wait for your employer to give you your PhilHealth ID Card and Member Data Record.

For Self-Employed Applicants

  • Step 1 – Prepare the documents needed in getting a PhilHealth ID.
    • two (2) accomplished copies of the PhilHealth Member Registration Form (PMRF)
    • at least two (2) valid IDs / photocopy of your Birth Certificate issued by the Philippine Statistics Authority (PSA)
    • two (2) latest 1×1 pictures
    • Barangay Certificate
  • Step 2 – Fill out the two (2) copies of the PhilHealth Member Registration Form (PMRF)
  • Step 3 – Go to Local Health Insurance Offices or PhilHealth express outlet nearest to you and submit your duly-accomplished and signed PMRF.
  • Step 4 – Wait for your MDR and PhilHealth ID’s processing, printing, and issuance. These may take a few minutes. Make sure to double check all the details.
  • Step 5 – Take note of your PhilHealth Identification Number. You will need your PhilHealth Identification Number in paying for your monthly contributions or filing your claim.

For Senior Citizen Applicants

  • Step 1 – Prepare the documents needed in getting a PhilHealth ID.
  • Step 2 – Fill out the two (2) copies of the PhilHealth Member Registration Form (PMRF) and attach your 1×1 photo to the form.
  • Step 3 – Present the identification card issued by the OSCA or any valid government ID at a PhilHealth office.
