The SSS offers a lot of loans and benefits for its members. That is one of the main reasons why most private companies in the country mandate their employees to be a member of the state-run social insurance institution.

To maintain an up-to-date account, members of the SSS must submit a monthly contribution. Accumulated contributions can make a member eligible to apply for the benefits and loans offered by the institution.

One of the benefits offered by the Social Security System is the SSS Sickness Benefit. It is designed to financially assist members who are unable to report to work for at least four (4) days due to sickness or injury.

There are requirements for the SSS Sickness Benefit application. The amount that a member will receive depends on his/her average daily salary credit. Here are the documents needed in applying for the benefit offer:

  • duly-accomplished Sickness Benefit Application (SBA) Form
  • SS ID/UMID card or any valid primary ID cards/documents, as follows:
    • Driver’s License
    • Passport
    • Voter’s ID Card
    • National Bureau of Investigation (NBI) Clearance
    • Postal Identity Card
    • Any other government-issued (secondary) ID cards/documents
  • Supporting medical documents for prolonged confinement/sickness, if any:
    • Laboratory, X-ray, ECG, and other diagnostic results
    • Operating room/clinical record that will support diagnosis

There are documents that are required for presentation during the processing of the claim. If the claim is processed by the member-borrower, an original of any one (1) of the member’s primary ID cards/documents or two (2) secondary ID cards/documents, both with signature and at least one with photo is needed.

If the claim is processed by an authorized representative of the member, an original of any one (1) of the member’s and the authorized representative’s primary ID cards/documents or two (2) secondary ID cards/documents, both with signature and at least one with photo must be presented.

There are steps if you want to apply for the SSS Sickness Benefit smoothly. In case the patient-borrower is not fully well to process the claim, it is best for the application to be done by a family member or an authorized representative of the patient to an SSS office.
