SSS Membership Requirements. In the case of employers, the requirements in registering for a membership depends on how your company is formed or operated. The application can be filed as a single proprietorship, a partnership, or a corporation.

- For single proprietorships, submit the following:
- SSS Form R-1 (Employer SSS Registration Firm)
- SSS R-1A (SSS Employment Report)
- For partnerships, submit the following:
- SSS Forms R-1 (Employer SSS Registration)
- SSS R-1A (Employment Report)
- photocopy of the Articles of Partnership
- For corporations, submit the following:
- SSS Forms R-1(Employer SSS Registration)
- SSS R-1A (Employment Report) signed by its President or any of the corporate officers
- photocopy of the Articles of Incorporation
SSS Member Registration. To register for a membership to the Social Security System, visit a branch of the state-run social insurance institution nearest to you and submit your documents. Wait for the processing. You will be given your SS number which will be used in your SSS transactions.
