REQUIREMENTS FOR DOST SCHOLARSHIP – Listed below are the documents required for application to an undergraduate studies scholarship.

Are you looking for a scholarship that can help you financially in getting through college? In the Philippines, one of the entities that you may turn to for scholarships is the Department of Science and Technology or more popularly called DOST.

The Department of Science and Technology has helped countless individuals in getting through college despite the poverty. Truth be told that the tertiary education requires thousands of money even if you are enrolled in a state college or university.

With regards to the scholarship offers of the DOST, among these are undergraduate scholarships under the DOST-SEI Merit Scholarship Program and the Science and Technology Scholarship Act of 1994 aside from the PhilFrance-DOST Scholarship. The latter is also known as the Republic Act No. 7687.

In applying for the offers of the state entity, there are documentary requirements for the DOST scholarship application that you must prepare and submit to the government office. It is important to prepare the documents needed to have them complete for submission within the application period.

Here are the documentary requirements for the DOST scholarship application:

  1. Certificate of Good Moral Character (Form C)
  2. Certificate of Good Health (Form D)
  3. Principal’s Certification
    • Applicant who is Grade 12 Graduating/Graduated student in the STEM strand (Form E1)
    • Applicant who is Grade 12 Graduating/Graduated student in NON-STEM strand and belongs to the upper 5% of the class (Form E2)
    • Applicant who is attending homeschool (Form E3)
    • Applicant who is under the Alternative Learning System (Form E4)
    • Applicant who graduated in High School before the K-12 program and belongs to the upper 5% of the graduating class (E5)
  4. Certificate of Residency (Form F)
  5. Parent’s Certification – Applicant has no pending immigration application to another country (Form G)
  6. DOST-SEI Scholarship Examination/Award (Form H)
  7. Applicant’s Certification of No Post-Secondary Units (Form I)
  8. Signed Declaration of Applicant and the Parent/Legal Guardian (Form J)
  9. Recent picture, passport size (4.5 cm x 3.5 cm or 1.8 inches x 1.4 inches)
  10. Birth Certificate
  11. Parent/s or Legal Guardian’s 2022 Income Tax Return / BIR Form 1701 / Employment Contract for OFW / BIR Certificate of Exemption from Filing of ITR / Certificate of Indigency / Certificate of Employment with Compensation / Proof of Pension

With regards to the first up to the eight requirements stated above, there are certain specific mandates from the DOST. You must implement the following instructions:

  • forms must be downloaded from the DOST website
  • print each form in an A4 size bond paper
  • forms must be accomplished by the proper authorities/ officials/ persons as indicated in each form
  • each form must be signed
  • scan or take picture of each accomplished form and save file
  • name each file: (Surname-Form __) when saving it
  • submit each form using the Upload Module in the E-Application System
  • use the appropriate upload field for each file
  • ensure that the files uploaded are correct and complete

With regards to the photo, the DOST also has some instructions. Here’s what you need to follow:

  • must show face of the Applicant, 80% of the picture
  • must be colored, with white background
  • with the name tag (approximately positioned 1 inch or 2.54 cm below the chin): Full name, handwritten- First Name, Middle Initial, Last Name, and Extension Name, if any (e.g. JUAN C. DELA CRUZ, JR.), and signature over printed name
  • taken within the last three (3) months prior to filing of application
